Confidentiality
Do not add identifiable information about a patient or any other person. That includes names, NHS or hospital numbers, dates of birth, addresses, photographs of people, and stories unique enough to identify someone.
Anonymise professional evidence before you add a file. Elaryvo asks you to confirm this immediately before you save. Supervisors and assessors only see what you share.
Logs and reflections are professional records, not a private diary. If you would not want anyone to read a thought, do not write it in Elaryvo. You do not have to reflect on every episode.
