Use these when another professional was present and saw or heard it.

| Mini-CEX | One patient encounter in any setting: history, examination, explanation or management. | A whole round, the take list, a procedure, or a discussion after the event. |
| Clinic | An outpatient appointment that was watched. | The same encounter on a ward or in ED — that is Mini-CEX. |
| Ward Round | The round as a whole: sequence, decisions, plans, team talk. | One patient on the round (Mini-CEX) or a spoken handover (SBAR). |
| Handover (SBAR) | A shift, board-round or escalation handover that was heard. | A clinic encounter, a letter, or the whole take. |
| Acute Take | The take or on-call list: prioritisation, flow and escalation across several patients. | One acute episode (ACAT) or one patient (Mini-CEX). |
| ACAT | One urgent or complex episode that was watched. | The whole take list, or a routine clinic or ward encounter. |
| LEADER | Leading and coordinating a team. | Clinical performance of one encounter, SBAR, or the take list. |
| DOPS | One procedure, watched from preparation to aftercare. | A logbook count. Use PBA for a fuller operative assessment. |
| PBA | A longer watched procedure, often in theatre. | A short DOPS. Still log the procedure separately. |
The assessor does not need to have watched the original activity.

| MSF | Views from several colleagues who have worked with you. | One observer of one event, or a supervisor report. |
| Supervisor Report | Performance over a posting or review period. | A substitute for watched encounters. |
| EPA / Entrustment | How much support an activity currently needs. | A Mini-CEX of a single encounter. |
| Local / other | Your hospital uses a name that is not in this list. Keep their title. | A named type that already fits. |
