A short guide for colleagues, nurses, AHPs, managers and others who may never have completed multi-source feedback before. No Elaryvo account needed.

You do not need to be an expert at this

A doctor has asked for an honest professional view of their work. You do not need an Elaryvo account. You do not need to write like an examiner. You do not need to comment on anything you have not seen.

Your name is not shown on the doctor’s report. Other people’s answers are not shown to you.

How to complete the form

  1. Open the link in the invitation email.
  2. Read the feedback focus so you know what period or role they mean.
  3. For each area, choose the description that matches what you have observed. If you have not seen it, choose “Not enough opportunity to comment”.
  4. Add a short comment where you have a concrete example. Optional comments still help.
  5. Say what they do well, then one thing that could make practice even better — only if you have something real to say.
  6. Do not include patient names, numbers, dates of birth or unique stories.
  7. Send the form. That is the whole task.

Prompts you can use

What did I actually see this doctor do?

When did they communicate clearly, or not?

How did they work with the rest of the team?

Would I trust them with this kind of work again?

What is one specific thing they could try next time?

What the ratings mean

Clear strength: you would trust this as a model for others.

Consistently effective: reliable in this area from what you have seen.

Generally effective: sound overall, with some room to grow.

Development would be helpful: a specific area to work on.

I have concerns: something that should be taken seriously. Urgent patient-safety issues still need the local clinical or governance process — this form is not a substitute.

Not enough opportunity: you have not seen enough to comment. This is a valid answer.
