How others experienced your work. Not always a formal assessment.

Feedback is not automatically an assessment

Feedback is information from another person about how your work was experienced or perceived. It may be formal or informal, individual, multisource, patient/carer or team-based.

Multi-team feedback

Use this when people in different professional groups worked with you in a shared area of practice. Record a de-identified feedback focus — never names or unique stories. Respondents are recorded by professional role, not by name on the report.

Patient or carer feedback

This is about the experience of receiving care or communicating with you: listening, respect, explanation, involvement, manner. It does not measure technical clinical competence.
