Never write names, numbers, dates of birth, addresses or other details that could identify a patient, family member, colleague or anyone else.
Logs and reflections are professional records. A supervisor, assessor or exporter may later read them. If you would not want anyone to read a thought, do not write it in Elaryvo. You do not have to reflect on every episode.
