No patient or colleague identifiers in uploads or free text.
Have identifiers been removed? Do you have the right to upload this file? Does it support the Evidence Record? Is this the correct version? Would structured information be enough without the full file?
Not every document needs redacting. Certificates usually do not. Patient documents, records about people you supervise, and complaints or incidents about someone else must be redacted first.
Confidential information does not belong in a career record.
Never write names, numbers, dates of birth, addresses or other details that could identify a patient, family member, colleague or anyone else.
Logs and reflections are professional records. A supervisor, assessor or exporter may later read them. If you would not want anyone to read a thought, do not write it in Elaryvo. You do not have to reflect on every episode.
